To register for your Ohio Medical Marijuana Card, the following requirements need to be evaluated including proof of residency and proof of a certifying medical condition:

You must have proof that you have been diagnosed with one of the certified medical conditions by providing medical records with your diagnosis documented or through records sent to us by your physician. View list of conditions >
If you are a veteran, you will receive a 20% discount on the cost of your medical marijuana card. Simply provide ID showing your Veteran designation to qualify for the discount which will be applied if you qualify for a medical marijuana card.
